To request a copy of your vaccination record please call 765-521-7059 and choose option 3. Leave your name, number, and request, and please allow 48 hours for us to return your call.
If you or your child did not receive their last vaccine at The Henry County Health Department, you will need to obtain the vaccination record at the facility that administered the last vaccine. All physician offices and clinics that administer Vaccinations can access the State's Immunization Program called CHIRP.
